1. Habits and Behaviour (i.e. exercising, clean eating etc)
2. Beliefs ( i.e. your beliefs about those habits and behaviours)
3. Identity (how you see yourself in relation to your goals)

The number 1 reason for this, is that the vast majority of fitness information and coaching is focused on helping people make changes to their habits and behaviour regarding exercise without addressing the beliefs and identity changes necessary for life long sustainable change.
